We’ll ask some questions about your needs and get started right away on finding the perfect aide. Or, you can fill out a contact form online with your name and phone number, your physician’s name and phone number, and if you are using it, your insurance information. We’ll call to follow up and get started on the intake process.
Once we have matched the right aide, we’ll do a more thorough intake interview and have a nurse do a patient assessment to create a personalized care plan.
A Registered Nurse from Margret Ultra Home Care will conduct an in-depth assessment of your present needs, medical history, and home environment, as well as your support system. Please have the following documents and information available: medical history, current medications, advance directives (such as healthcare proxy, power of attorney, and living will), contact person(s), and physician information.
Using the information gathered from the assessment, the nurse will develop a personalized care plan that your aide will follow.
