A Registered Nurse from Margret Ultra Home Care will conduct an in-depth assessment of your present needs, medical history, and home environment, as well as your support system. Please have the following documents and information available: medical history, current medications, advance directives (such as healthcare proxy, power of attorney, and living will), contact person(s), and physician information.
Using the information gathered from the assessment, our nursing team develops a personalized care plan that your aide(s) follow.
